I know that speaking about anything other than home hemodialysis is frowned upon. I get it. We went from in-center hemodialysis to home hemodialysis. I am supposed to say, “Well, the good news is the patients are at home.” I can’t take that stance. All home modalities aren’t equal. There is residual renal function, urine output, mortality, and, most importantly, quality of life to factor into the equation. We want to discuss the ease of pulling a heavy machine through the airport on wheels and the ease of checking unlimited medical supplies with a doctor's note at no charge. That’s because one modality has our feed filled with promotion from patient advocates, while peritoneal dialysis doesn’t have a single player in the game. Our patient sent us this photo of them bringing four dialysate bags in their carry-on and borrowing a bellhop to hang a bag to do their one CAPD exchange without a cycler for their weekend trip. If you had a choice of one single dose exchange (QLPD) that had a 10-minute gravity fill followed by a 10-minute gravity drain daily without a cycler with diuretic support OR four times a week with 15G needles (but only 2 hours running and 45 minutes setup and clotting) What would you choose? You would choose what your clinical team wanted to discuss with you.
